Dr.Sunil Phol personal experience by undergoing Full Mouth Reconstruction:

When Dr. Sunil was a first year Dental student at Chiangmai University, he met with an accident where his lower jaw was broken and his top front teeth were fractured, with two molars also broken. He went through6 Root Canal treatment, 6 Crowns and many screws were fixed in his jaw. The treatment took years to complete. It was always in his mind when he was a dental student to find out ways to do full mouth Reconstruction in a short period of time. Today, Dr. Sunil International Dental Center provides a customized treatment plan tailored to your Budget and travel plan. A full mouth restoration (or sometimes called reconstruction or rehabilitation) is individual restructuring of each and every tooth in a mouth. FMR is called for where the teeth are worn down, broken down or missing, causing problems in both the bite and appearance of the mouth. The most common solution to these problems is crown, bridges, root canal, implant work. Full Mouth Reconstruction in the hands of Dr. Sunil is usually so successful that others cannot discern the real teeth from the restoration. This technique involves some easy preliminary information gathering. Reconstruction is completed in just 3-5 appointments.

During the preparation appointment all old restorations and any decay are removed, deep broken tooth are fixed by Root Canal, impressions are made of the underlying healthy tooth structure and temporary teeth are made. These temporaries are worn for few days until the Crowns are ready.

FMR is designed to restore your smile, and make eating easier and more comfortable. Cosmetic dentistry has evolved dramatically in recent years. A poor bite can cause jaw joint problems, muscle tenderness, and even headaches. Chewing and speech may be affected as well, so FMR will fix all the problems.

1st Option

Patients who still have their teeth left but they are in bad conditions the word down deep decay, de-colorization or not proper alignment; these can be fixed by Crowns and Veneer.

One can start with these options:

  • Top jaw 6 crowns or veneer + whitening
  • Top jaw 6 crowns or veneer + lower jaw 6 crowns or veneer
  • Top 10 jaw crowns or veneer + lower jaw 10 crowns or veneer

These procedures do not take more than a week within 2-3 visits and not more than 2-3 hours per visit. Each crown will be customarily crafted and after the treatment, a great smile is back dazzling. With the option, one can always come back and do the lower jaw or increase the numbers of crown in future.

2nd Option

If you are missing out all of your teeth, a set of full denture or implant support full bridge can replace them. Dental Implants will replace both your lost natural teeth and some of the roots. Dental Implants provide several advantages over other teeth replacement options. In addition to looking and functioning like natural teeth, implant-supported full bridges or dentures are designed to be long lasting. Implant-supported full bridges and dentures are also more comfortable and stable than conventional dentures, allowing you to retain a more natural biting and chewing capacity.

In the long term, implants can be more esthetic and easier to maintain than conventional dentures. The loss of bone that accompanies conventional dentures leads to recession of the jawbone and a collapsed, unattractive smile. Conventional dentures make it difficult to eat certain foods.

How will the implants be placed?

First, implants which look like screws or cylinders are placed into your jaw. Then, over the next six months, the implants and the bone are allowed to bond together to form anchors for your artificial teeth. During this time, a temporary teeth replacement option can be worn over the implant sites.

Often, a second step of the procedure is necessary to uncover the implants and attach extensions. These temporary healing caps, along with various connecting devices that allow multiple crowns to attach to the implants, complete the foundation on which your new teeth will be placed. Your gums will be allowed to heal for a couple of weeks following this procedure.

Depending upon the number of implants placed, the connecting device that will hold your new teeth can be tightened down on the implant, or it may be a clipped to a bar or a round ball anchor to which a denture snaps on and off.

Finally, full bridges or full dentures will be created for you and attached to small metal posts, called abutments, or the connecting device. After a short time, you will experience restored confidence in your smile and your ability to chew and speak. Every case is different, and some of these steps can be combined when conditions permit. Your dental professional will work with you to determine the best treatment plan.

Full mouth reconstruction

Full Mouth Replacement Option:

A. Full Denture 2 jaw

Impression of both jaws will be taken and sent to the lab to proceed. Patients will be coming to the clinic once or twice a day for 30 minutes for trial and denture will be ready not more than 3-5 days.

B. Over denture 2 jaw with 4 Dental Implant + 4 locator

This option first the Implant will be placed on both the jaws. You will go back with a temporary denture replacement within 3-5 days and after 4-6 months, the locator will be placed with a new denture which is removable but strong and have a good grip on jaw and patient can remove the denture to clean which is a very good option for denture with implant. The second trip will take around 5-7 days.

C. All on 4 Implant

All on four treatments provides a permanent, screw-retained replacement for the entire upper or lower set of teeth. The procedure is best for patient with significant tooth loss or decay and for people whose bone loss in jaw area which poses the problem of reconstruction of the jaw bone.

The all on 4 treatment concept consists of four titanium implant. These implants act as “pillars” that supports the denture with at least 12 teeth fixed in the jaw. So basically, it’s a “fixed denture with 4 Implant”.

Procedures:

1st Trip

Placing of 4 Implants on each jaw. This procedure will take 3-5 days. Patient goes back with a temporary denture.

1st Trip

After 4-6 months, fixing permanent denture to the implant (7 days)

** This procedure can be done in one visit also which is called “teeth in an hour” but the chance of failure is high. At Dr. Sunil Clinic, we prefer to do in 2 visits giving the time for the implant and gums to heal properly so the denture is fixed esthetically. The weak part of this procedure is patient still have a denture which sometimes food gets accumulated under the denture and causes bad breath. In the past, All on 4 was a very common treatment but today’s time after seeing the result of food accumulating underneath the gums and lack of follow-up for International Patients. Dr. Sunil prefers doing Over-denture, 2 jaws with 4 Dental Implants + 4 Locator and if patient’s bone structure is good, we will suggest doing six implants with 10-12 unit permanent porcelain bridge.

D. All on 6 Implants

E. Top jaw 6 Implants with 10-12 unit permanent porcelain bridges and lower jaw 6 implants with 10-12 unit permanent porcelain bridges.

This option is for patient has lost all their teeth and bone structure is good.

Procedures:

1st Trip

Top and bottom jaw placement of 12 Implants. This procedure will be done under local or general anesthesia. If local anesthesia, we can provide sedative to calm the patient and the treatment can perform at the clinic only within 2 hours time. If patient wants to get the surgery done under general anesthesia, patient will be admitted in a private hospital for one night with additional cost of $1,000. Patient will go back with temporary dentures and wait for healing time for 4-6 month.

2nd Trip:

After 6 month, 10-12 unit permanent bridge will be placed on the 6 Implant which is on each jaw. This procedure will take 7 days and patient will go back with full permanent porcelain bridge which gives the most esthetic and functional.
